PATIENT CENTERED MEDICAL HOME

What is a Patient Centered Medical Home?

A Patient Centered Medical Home (PCMH) is a medical office where a team of healthcare providers work together to provide a new type of care to patients. PCMH is based on a continuous and personal relationship between a patient, provider, and the patient’s care team.

What Should Your Care Team Do?

Get to know you: Your care team should get to know you, your family, your life situations, and your cultural values and use this information at each visit to suggest treatments plans that will make sense to you.

Support you in caring for yourself: It’s important to leave the office with a clear idea of how to care for yourself. Your care team should help you set goals for your care and help you meet those goals one step at a time.

Your care team can provide you with information about classes and educational opportunities to learn more about your condition and help you stay healthy. Your providers will also coordinate your care with other health providers to ensure you are receiving proper care. They will also provide you with evidence-based care.

What Can You Do?

Learn about caring for yourself: You are an active partner in your own care and you should take time to get to know your care team, learn about your condition, and set goals alongside your provider. It is important to follow the plan that you and your medical home team have agreed upon to get the desired results and improve your health.

Communicate with your care team: You should select a personal provider and always bring a list of questions to each appointment. You should also bring a list of any medications, vitamins, or supplements you’re taking and always tell your care team when you don’t understand something that was said.

To better coordinate care, inform your care team when you receive care from another health professional including: tests, hospitalizations, ER visits, and specialists. Your care team is there for you so it’s important to always talk openly and express any needs or concerns to improve your level of care.

How a Medical Home Works for You:

  • Better access to care when you need it, 24 hours a day, 7 days a week
  • Reminders for important preventative and chronic care services
  • Enhanced coordination with other providers involved in your care
  • More education and self-management support
  • Individualized care plans to help meet your goals

TELEMEDICINE

TELEMEDICINE BY OUR DOCTORS

Emmanuel Family Clinic-Saluda has added telemedicine to its primary care offerings.

We are pleased to offer convenient telemedicine solutions to help connect patients and healthcare providers to ensure the continued delivery of high-quality care.

As the situation with COVID-19 continues to evolve, telehealth enables our clinical team to practice social distancing to further reduce the spread of illness while still meeting our patients’ health needs. We look forward to the many ways this new offering will further advance our mission of Making Communities Healthier.

For patients who meet certain clinical criteria, Emmanuel Family Clinic-Saluda is offering two types of telehealth visits: telephonic and tele-video. A telephonic visit is simply a patient phone call with a provider (when they cannot connect through our secure video system), and a tele-video visit is a virtual, face-to-face visit with a healthcare professional using a video conferencing service.

Patients can request a telehealth visit by calling our office at (864)445-2250, just as they would for an in-person visit. If the virtual visit is deemed clinically appropriate, the patient will be given an appointment time and instructions for the best way to connect given the available platforms. Then, instead of coming to the office, he or she would call back at the scheduled time and be “checked in” by a nurse or office manager, and then transferred to the provider for the call or two-way video.

A few restrictions on telephonic visits may apply, including:

  • They cannot be utilized to treat patients for a condition that the patient has been seen for in the previous seven days
  • They cannot be used to treat a condition that the patient is already coming in for within the next 24 hours.

Patients who are concerned they may be experiencing symptoms of COVID-19 are encouraged to consider telemedicine appointments to help further reduce the spread of the respiratory illness. Leveraging telemedicine also conserves personal protective equipment (PPE) and other clinical resources that are needed when treating a patient with suspected COVID-19 in the clinic or hospital setting.

TELEMEDICINE BY MUSC SPECIALISTS

The Medical University of South Carolina in coordination with Emmanuel Family Clinic Saluda offers telehealth appointments with specialists such as psychiatrists, nutritionists, and others. You can be seen by a specialist in our office through tele-video.

The appointment is made after you are evaluated by our primary care providers.

Our patients have been blessed with this service which offers a specialized care close to them through telehealth.
